Dear Sirs,
Please by kindly informed that as fees are paid to State Tax Inspectorate (STI) under the Ministry of Finance of the Republic of Lithuania there is strict order from their part.
If you would like to refund overpaid or paid by mistake fees for marketing authorization/renewal, all kind variations or the changes you need to submit to State Tax Inspectorate under The Ministry of Finance of the Republic of Lithuania these documents: - Form FR0781;(note: form FR0781 is only in Lithuanian and all covering and explanatory letters are only in Lithuanian too. So you need someone to help you to fill this form);
- copy of account statement (the same was sent to SMCA with AF);
- the confirmation letter (original) from State Medicines Control Agency of Lithuania (i.e. from us) about refund.
To get a confirmation letter (it will be written in Lithuanian) you need to send to us (to SMCA) official letter (it could be in English) about the refund and copy of account statement.
If there is authorised juridical person in LT, the confirmation letter’s original will be send to it for further contact with STI.
It there is no authorised juridical person in LT, the firm for refund must send filled FR0781 original and copy of account statement to SMCA and SMCA will have contact with STI.
